Our Theory of Change
We are developing a Theory of Change that will guide everything we do. It’s our roadmap for understanding how our work creates meaningful change for adults living with long-term health conditions.
By setting this out clearly, we can:
Focus on what matters most — helping people thrive in work and community life.
Identify what we need to measure to show progress and impact.
Learn from what’s working (and what isn’t) so we can keep improving.
This is an ongoing process. Over the coming months, we will be inviting input from the people who matter most — adults with lived experience, our partners and supporters. Their perspectives will help us make sure the Theory of Change truly reflects the challenges people face, the opportunities we can create together and the role of wider systems change in enabling inclusive employment and wellbeing.
Our Theory of Change will also shape our new impact measurement framework, giving us the data and insights we need to stay accountable, influence change and strengthen our fundraising.
